    ​If you would like to play on a team with a "buddy", please list their name below.  In the interest of team parity, we are limiting the request to 2 players.  (Relatives are exempt from this rule - siblings, father/son, etc.)
    ​    We will try to accommodate your request...but it is not guaranteed.

    Release of Liability Agreement & Acknowledgment of Risk

    I UNDERSTAND AND AGREE THAT THE SPORT OF ICE HOCKEY HAS INHERENT RISKS OF PHYSICAL INJURY, INCLUDING SERIOUS BODILY INJURY, PERMANENT DISABILITY, PARALYSIS, AND DEATH, AND I ACCEPT AND ASSUME ALL SUCH RISKS.  THESE RISKS AND DANGERS MAY BE CAUSED BY MY OWN ACTIONS OR ACTIONS OF OTHERS. I AGREE TO WEAR ALL PROTECTIVE HOCKEY EQUIPMENT INCLUDING SHOULDER PADS AND A HELMET.  IF I CHOOSE NOT TO WEAR A FULL FACE SHIELD, I ASSUME ALL LIABILITY SHOULD I SUSTAIN AN INJURY.

    Acknowledgment of Risk
     
    I hereby acknowledge and agree that use of Meramec Hockey Club / FSI Shark Tank facilities and services and participation in Old Shark Hockey Club (OSHC) programs and activities come with inherent risks. I have full knowledge and understanding of the inherent risks associated with such use and participation in OSHC programs, activities and services, including but in no way limited to: (1) slips, trips, and falls, (2) injuries, (3) athletic injuries, and (4) illness, including exposure to and infection with viruses or bacteria. I further acknowledge and agree that the preceding list is not inclusive of all possible risks associated with such use and participation and that the preceding list in no way limits the operation of this Acknowledgment.
     
    Coronavirus / COVID-19 Warning & Disclaimer

    Coronavirus (“COVID-19”) is an extremely contagious virus that spreads easily through person-to- person contact. COVID-19 infections have been confirmed in Missouri, including St. Louis County. Federal, State and Local Authorities recommend social distancing as a mean to prevent the spread of the virus. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in OSHC programs and activities or accessing Meramec Hockey Club / FSI Shark Tank facilities could increase the risk of contracting COVID-19. OSHC in no way warrants that COVID-19 infection will not occur through participation in OSHC programs and activities or as a result of accessing or using Meramec Hockey Club / FSI Shark Tank facilities and services.

    Waiver, Release, Indemnification & Covenant Not to Sue 

    In consideration to participate in OSHC programs and activities, I, on behalf of myself, my heirs, representatives, executors, administrators, and assigns, HEREBY DO RELEASE OSHC, Meramec Hockey Club, its affiliate and related entities, and its officers, directors, employees, volunteers, agents, representatives and insurers of them, (collectively referred to as “Releasees”) from any causes of action, claims, or demands of any nature whatsoever including, but in no way limited to, claims of negligence, which I, my heirs, representatives, executors, administrators, assigns and/or children may have, now or in the future, against Releasees on account of personal injury, property damage, death or accident of any kind, arising out of or in any way related to the use of Meramec Hockey Club / FSI Shark Tank facilities, services or equipment, or participation in OSHC programs or activities, whether that participation is supervised or unsupervised, however the injury or damage occurs, including, but not limited to the negligence of Releasees.
    In consideration of access by me to Meramec Hockey Club / FSI Shark Tank facilities and services and/or participation in OSHC programs and activities, I, the undersigned, agree to INDEMNIFY AND HOLD HARMLESS Releasees from any and all causes of action, claims, demands, losses, or costs of any nature whatsoever arising out of or in any way related to such access and or participation by me.
    I hereby certify that I have full knowledge of the nature and extent of the risks inherent in such access and participation and that I am voluntarily assuming said risks. I understand that I, not the OSHC or any Releasees, will be solely responsible for any loss or damage, including personal injury, property damage, or death, I sustain while accessing Meramec Hockey Club FSI Shark Tank facilities and services and/ or participating in OSHC programs and activities and that by signing this Agreement I HEREBY RELEASE OSHC and all other Releasees from all liability for such loss, damage, injury or death.

    I further certify that I am in good health and that I have no conditions or impairments which would preclude my safe use of Meramec Hockey Club / FSI Shark Tank facilities and services and/or participation in OSHC programs and activities.

    I further certify that I am over the age of 18, and that I am therefore of lawful age and otherwise legally competent to agree to this Acknowledgment of Risk on my behalf. I further understand that the terms of this Acknowledgment of Risk are legally binding and certify that I am signing this Agreement, after having carefully read it, of my own free will.
